Two versions of the Forms are available.
1. Word document (.doc) .
2. PDF files. Requires Acrobat Reader installed on computer
to open them.
The advantage of the .doc is you can
save them on your computer and type in the entries.
With PDF files, you can only read them or print them out
and fill in the blanks by hand or with a typewriter.

Suggestion: (A quick way to save these forms
to your computer)
1. Right click on the link of the desired form
below.
2. From the pop-up menu, select "Save As...." or
"Save Target As....".
3. Navigate to the desired file location on your
computer and select it.
4. Click on "Save".
Using this method, the desired forms will be saved almost instantly on
your computer without needing to wait for them to open on the Internet.
You can then go to the file location on your computer and open them.
They will open faster on your computer than on the Internet. |
